Washington Consumer Health Data Privacy Policy

Scope of this Policy
If you reside in the State of Washington, this policy supplements Neuro Support Service’s Privacy Policy and applies to “consumer health data” as defined by the Washington State My Health My Data Act (“Consumer Health Data”). Terms not defined in this policy have the meaning ascribed to them in our Privacy Policy.
Categories of Consumer Health Data That NEURO SUPPORT SERVICES, LLC may collect:

The categories of personal data NEURO SUPPORT SERVICES, LLC collects are described in the Personal Data We Collect section of our Privacy Policy. Because Consumer Health Data is defined very broadly in the Washington My Health My Data Act, depending on the context of your interactions with our Platform, several of the categories of personal data we collect are or could be considered Consumer Health Data. Examples of Consumer Health Data may include information such as:
a. Information about health-related conditions, symptoms, diagnoses, or treatments that you provide to us through surveys or other communications with Us regarding community health.
b. Photographs, which may be considered biometric information under the Washington My Health My Data Act.
c. Information that could identify your attempt to seek health care services or information, including information on our Platform about health issues or conditions.
d. Other information that may be used to infer or derive data related to the above or other health information.
Sources of Consumer Health Data
We collect Consumer Health Data from the sources further described in the Personal Data We Collect section of our Privacy Policy, which may be from you or automatically when using our Platform.
Why We Collect and Use Consumer Health Data
While we don’t intentionally collect Consumer Health Data, we may come into possession of Consumer Health Data through your use of our Platforms. Where we do, we use Consumer Health Data for the purposes described in How We Use Your Personal Data section of the Privacy Policy.
NEURO SUPPORT SERVICES, LLC collects and uses Consumer Health Data as reasonably necessary to provide you with the services you have requested or authorized. This may include personalization of certain membership opportunities and other essential business operations that support the provision of NEURO SUPPORT SERVICES, LLC’s Platform and related services to you, our members, our Directory users, and other visitors to our Platform.
For example, you may choose to share Consumer Health Data with the community on your Directory listing. We may use Consumer Health Data for other purposes for which we give you choices and/or obtain your consent as required by law – for example, for advertising or marketing purposes. See the How To Exercise Your Rights section below for more information on the choices you may have.
Our Sharing Of Consumer Health Data
We may share each category of Consumer Health Data described above in the Categories of Consumer Health Data That NEURO SUPPORT SERVICES, LLC May Collect section for the purposes described in the How We Use Your Personal Data section of the Privacy Policy. For example, We may share Consumer Health Data with your consent, or as reasonably necessary to complete any transaction or provide any services you have requested or authorized, and with our service providers and agents that help Us with any of the uses as described in the previous section above.
In addition, We may share Consumer Health Data with third parties when you tell us to do so, such as when you give consent to post it for marketing and advertising purposes. And We may disclose Consumer Health Data when we believe doing so is necessary to comply with the appliable law or
response to valid legal process.
Third Parties Who May Receive Consumer Health Data
As necessary for the purposes described above, We share Consumer Health Data with the following categories of third parties, processors, and affiliates:
a. Service Providers. Vendors or agents (“processors”) working on our behalf may access
consumer health data for the purposes above. For example, companies we’ve hired to provide
customer service support or assist in protecting and securing our systems and services may need
access to data to provide those functions.
b. Business partners. We may share consumer health data with other companies, for example,
where you use a service that is cobranded and jointly operated with another Neuro Support Services, LLC, or where
you use our services and interact with another Neuro Support Services, LLC.
c. Financial Institutions & payment processors. When you make a purchase or enter into a
financial transaction, we will disclose payment and transaction information to banks and other
entities as necessary for payment processing, fraud prevention, or other related financial
services.
d. Affiliates. We enable access to data across our subsidiaries, affiliates, and related companies
e. Other third parties. In certain circumstances, it may be necessary to provide data to other third
parties, for example, to comply with the law or to protect our rights or those of our customers.
f. The public. You may select options available through our Platform’s and related services to
publicly display and disclose certain information, such as your profile, demographic data, and
content and files, which may include Consumer Health Data.
How to Exercise Your Rights
Under MHMDA Washington residents have the right to access, delete, or withdraw consent as it relates to Consumer Health Data, subject to certain exceptions. Depending on the Consumer Health Data you choose to share with Us, you may be able to view or take action regarding your Consumer Health Data through your account settings. For example, through your account settings, you can limit the visibility of some of your activities, request to exercise certain of your rights, and view your activity, public contributions and your communications with businesses. If you request to exercise a right under the Washington My Health My Data Act is denied, you may appeal that decision by contacting mash@neuross.org.
If your appeal is unsuccessful, you can raise a concern or lodge a complaint with the Washington State Attorney General at www.atg.wa.gov/file- complaint
